If you reside in Winnebago County, you have access to free transportation to and from our facility in Rockford, IL.
We can provide a car seat upon request and accommodate vehicles for wheelchairs. A parent or guardian may accompany the child or young adult if needed.

Step 1: Call the number on the back of your insurance card at least 5 days before your visit to arrange your insurance covered transportation.

Step 2: Tell them that you need to schedule a ride for a healthcare visit.

Step 3: Follow the steps provided by your plan to schedule and confirm the ride.

For Patients with Illinois Medicaid 

Step 1: At least 5 days before your visit, call First Transit at (877)725-0569 to receive a list of transportation companies to call for your insurance covered transportation.

Step 2: Call the transportation companies to find one that is available.

Step 3: Call First Transit back to get the trip approved and receive a reference number.

Step 4: Call the transportation company back with the reference number to confirm the ride.
